Erie County Health Department Releases Five-Year Community Health Improvement Plan

ERIE — The Erie County Department of Health has published its 2026-2030 Community Health Improvement Plan, building on findings from the county’s 2025 Community Health Needs Assessment.

The plan identifies actions intended to address community health needs and improve population health. It is designed to serve as a resource for implementation through collaboration among community organizations and partners.

The department plans to update the document annually with new health metrics and a summary report.
